All Staff must submit their time off request online. Due to the design of MHG’s PTO request form (free text rather than selection), E & D staff are required to fill out both forms in order to populate the time off table in a consistent format that makes it easy for you to sort by date.
C staff time off request form
Start date: the day your time off starts (can’t be before 8:30 AM)
End date: the last day of your time off (can’t be after 5:30 PM)
E & D staff time off request form
If your badge ID starts with letter CE or D, fill out the form below (in lieu of MHG’s PDF form)
NPs/PAs only: MD approval and internal coverage must be secured before submitting the request. OM will approve/deny based on request calendar, with the assumption that MD approval and coverage have been secured. Otherwise, approval may be withdrawn at any time.
