Schedule, reschedule or cancel an appointment

We recommend using the appointment tool (MY Appointment button) on any page of our website to request appointment scheduling, rescheduling or cancellation. Our front desk staff will review your requests and contact you to confirm, or offer closest alternatives.

New Patients

We are accepting new patients through professional or self referrals. We do need a copy of the past medical records to establish your service at our office. You can request a copy directly from your other physicians, or sign a record request form at our office so that our staff can contact your other physicians. We have direct access to all Memorial Hospital and Memorial physicians clinics electronic records.

No Show

If you will not be able to make it, We ask that you notify us at least 48 hours before your appointment so that we can serve another patient in need of healthcare.

What to bring?

Please bring your actual medication containers to each and every office visit. Our nurses are going to go over your medication every time.

Asking questions

We recommend that you write down your questions before you come to your visit. You will have an opportunity to ask all kinds of relevant questions when you are seen by your cardiologist, nurse practitioner, nurse, and medical assistant. Our nurses will go over your visit before you leave to make sure your questions are answered.

Duration of the visit

The duration of the visit can vary widely. We aim to complete your visit within 30 minutes. First visit may take longer.

What tests would I have to have?

Deciding of which test to order depends on your medical condition. An essential test that all new patients have is Electrocardiogram (ECG) done at our office. A wide specs=trum of test are available to help diagnose heart disease and monitor response to treatment. See our home page.

Where are the tests performed?

Diagnostic test are done at our clinic or Memorial Hospital. Event monitors are devices you wear for a defined period of time. Lab test can be sent to the lab of your choice.

Will I be contacted with test results?

depending on the test, you will be contacted if the test results are abnormal. In most cases, a follow up appointment will be schedule to go over the result with your physician or nurse practitioner, and adjust the treatment plan accordingly

How can I apply?

However, Healthcare providers with two or more years of automomous clinic experience are encouraged to contact the office manager directly, even before submtting an application.

Who is the employer?

There are two general types of positions, clinical and clerical.
Clinical positions, such as medical assistants and nurses are hired under Memorial's payroll. Clerical positions are under Coast Heart Institute's payroll but will also have to go through MHG's emploayment process.
Nurse practitioners and physicain assistants can be hired by MHG or CHI. Physician's can only be hired by Coast Heart Institute.

I have applied but have not heard back from you. What should I do?

We usually receive a large number of applications to our open positions. We cannot posibly call or email every applicant. We have created an online job application stats check tool. Applciants must use that form online to see the status of their application. We update the information as frequently as possible. You need to check the status at least once a day.

The aplications that do not meet the position requirements will not pass through to the reviewers
Even after the match process we oftentimes end up with a considerable number of applications. It is possible that we may miss some. If you reasonably believe taht you are one of the strongest candidates, take further steps.
late applications generlly have less exposure. We can usually give more time to each application when th ejob posting is fresh and the number of application to review is still small. We encourage you to sign up to recieve our mailing list and apply as soon as a position is open.
